Intake Mental Health Screening

This program ensures every new arrival is screened by a trained Qualified Mental Health Professional (QMHP) for prior mental illness, trauma, and substance-use history. Standardized tools such as the CMHS-M/F and Columbia-Suicide Severity Rating Scale (C-SSRS) allow staff to quickly identify at-risk individuals for immediate follow-up.

Comprehensive Mental Health Evaluation

For inmates who screen positive, a QMHP performs a full one-to-one evaluation within 30 days, reviewing psychiatric history, trauma, medications, and current symptoms. These structured assessments form the basis of individualized treatment plans that guide ongoing care and psychiatric referral.

Individual and Group Counseling

Counseling services combine trauma-informed individual therapy with evidence-based group interventions for coping, relapse prevention, and emotional regulation. Sessions promote stability, insight, and long-term behavioral improvement while supporting continuity of care after release.

Psychoeducational & Specialized Groups

These group programs teach inmates practical skills for managing stress, anger, and mental-health symptoms, while increasing understanding of their conditions. Specialized sessions—such as trauma recovery and self-esteem groups—reinforce self-awareness and emotional resilience.

Community Mental Health Coordination

Wexford partners with local agencies to ensure inmates receive consistent care before, during, and after incarceration. This collaboration promotes seamless reentry and reduces recidivism among individuals with behavioral-health needs.

Discharge Planning/Re-Entry Services

Wexford Health’s comprehensive discharge/re-entry program focuses on assessing patients and tailoring individualized plans to help them become productive, law-abiding citizens. We provide each patient with a discharge plan that includes information about local health care resources and recommendations for obtaining ongoing care upon release.
